NVREST Inj. is an intravenous or intramuscular injection containing Ondansetron, a selective serotonin 5-HT3 receptor antagonist. It is primarily used to prevent and treat nausea and vomiting associated with chemotherapy, radiation therapy, surgery, or other medical treatments.
Typical Dose for Adults:
- Chemotherapy and Radiation:
-- IV/IM injection: A typical dose is 8 mg of Ondansetron administered 30 minutes before chemotherapy or radiation treatment, followed by additional doses as needed.
- Postoperative Nausea and Vomiting (PONV):
-- IV/IM injection: 4 mg administered before or immediately after surgery, with additional doses as required.
- o Maximum Daily Dose: For most indications, the typical daily dose should not exceed 32 mg.
Administration:
- IV administration: Inject slowly over 2 minutes.
- IM administration: Direct injection into a muscle.
- Note: Dose adjustments may be needed for elderly patients or those with hepatic impairment.
Hypersensitivity: Avoid in patients with known hypersensitivity to Ondansetron or any of its components.
Cardiac Issues: Caution is advised in patients with electrolyte imbalances, congenital long QT syndrome, or those receiving medications that affect the heart’s rhythm, as Ondansetron can prolong the QT interval.
Serotonin Syndrome: Rare, but possible when combined with other serotoninergic medications (e.g., SSRIs, SNRIs). Symptoms include agitation, confusion, fever, shivering, and muscle twitching. Immediate medical attention is needed if these symptoms occur.
Pregnancy and Breastfeeding: Ondansetron is classified as a Category B drug during pregnancy, meaning it should be used during pregnancy only if clearly needed. It is excreted in breast milk, so it should be used with caution in breastfeeding mothers.
Hepatic Impairment: The dose should be reduced in patients with severe liver impairment (Child-Pugh Class C).
