Baclofen

Baclofen is a gamma-aminobutyric acid (GABA) receptor agonist, which helps inhibit the transmission of nerve signals responsible for muscle contraction. By targeting specific receptors in the brain and spinal cord, baclofen effectively reduces excessive muscle stiffness and spasm, allowing for better mobility and comfort in individuals with certain neurological conditions.

Indications:
  • Muscle Spasticity: Baclofen is used to treat muscle spasticity caused by conditions such as: Multiple sclerosis (MS), Spinal cord injuries or diseases, Cerebral palsy.
  • It helps reduce muscle stiffness, spasms, and improves muscle control.
  • Baclofen may also be prescribed off-label to treat conditions such as muscle spasms or back pain, but this is less common.
Dosage and Administration:

Initial dose: Typically, 5 mg three times a day, which can be gradually increased to a maximum dose of 80 mg per day divided into multiple doses, depending on the patient's response and tolerance.

Precautions:
  • Gradual Discontinuation: Baclofen should not be stopped abruptly as this can lead to withdrawal symptoms such as hallucinations, seizures, and increased spasticity. Tapering the dose gradually is essential under medical guidance.
  • Kidney Function: Baclofen is primarily excreted by the kidneys, so renal impairment can lead to increased drug levels in the body. In patients with kidney disease, lower doses may be required, and kidney function should be monitored.
  • CNS Effects: Baclofen can cause sedation, drowsiness, dizziness, or confusion. Patients should avoid activities requiring full alertness (such as driving or operating heavy machinery) until they know how the drug affects them.
  • Pregnancy and Lactation: Baclofen should be used with caution during pregnancy (Category C) and breastfeeding because the safety of the medication during these periods has not been fully established. It should only be used when clearly needed and prescribed by a doctor.
  • Severe Side Effects: In rare cases, baclofen can cause muscle weakness, hypotension, or respiratory depression, especially at higher doses or in combination with other sedatives.
