Levetiracetam

Levetiracetam is an anticonvulsant (anti-seizure) medication used to treat epilepsy and prevent seizures. It is often prescribed to patients with partial seizures, generalized seizures, and myoclonic seizures. Levetiracetam works by stabilizing electrical activity in the brain, reducing the frequency and severity of seizures.

  • Recommended Usage
Indications:
  • Partial seizures: For adults and children aged 1 month and older.
  • Generalized tonic-clonic seizures: For both adults and children with generalized epilepsy.
  • Myoclonic seizures: Used in patients with juvenile myoclonic epilepsy.

Levetiracetam is often used as adjunctive therapy (in combination with other seizure medications) or as a monotherapy (alone).

Dosage and Administration:
  • Adults: The typical starting dose is 500 mg twice daily, which can be gradually increased. The usual maintenance dose is between 1000 mg and 3000 mg per day, depending on the patient's condition.
  • Children: The starting dose varies based on age and weight, but generally ranges from 10 mg/kg/day to 60 mg/kg/day.
  • Levetiracetam is available in oral tablets, oral solution, and intravenous (IV) form.
Precautions:
  • Kidney function: Levetiracetam is primarily excreted through the kidneys. Dose adjustments are needed for patients with renal impairment.
  • CNS side effects: It can cause drowsiness, dizziness, irritability, or mood changes. Patients should be cautious when driving or operating heavy machinery.
  • Psychiatric effects: Rarely, it can cause depression, agitation, anxiety, or even suicidal thoughts, so it’s important to monitor for any mood or behavior changes.
  • Pregnancy and breastfeeding: It is classified as Category C for pregnancy, meaning it should only be used if the benefits outweigh the risks. It is excreted in breast milk, so caution is advised when breastfeeding.
Contraindications:
  • Hypersensitivity to levetiracetam or any of its components.
  • Severe renal impairment: Dose adjustments are required for patients with significant kidney issues.
Key Benefits:
  • Effective seizure control: Levetiracetam is effective in controlling various types of seizures, including partial and generalized seizures.
  • Well-tolerated: It is generally well-tolerated and has fewer drug interactions compared to other anticonvulsants.
  • Flexible dosing: Available in oral and intravenous forms, making it suitable for a wide range of patients, including those unable to take oral medications.
Side Effects:
  • Common: Drowsiness, dizziness, fatigue, headache, and irritability.
  • Serious: Psychiatric symptoms (e.g., depression, aggression, mood swings), allergic reactions, and suicidal thoughts.
  • Rare: Severe skin reactions like Stevens-Johnson syndrome.
